How do I nominate fan art?

I'm organizing my ballot for the nominations but I've ran into a problem.  How do I nominate fan art?  Do I list the artist or the link to the fan art?  Non of the fan art have titles so I'm wondering how to do about this.  Please help me out!

Try listing the author as well as the URL, with maybe a little description of the pic...


e.g.: Bob Smith's Ray Tracer pic: www.raytracerrocks.com


(note: this site doesn't exist. ;))
